What was studied?

This study asked whether the subgingival oral microbiome of severe periodontitis differs by sex. Researchers sampled subgingival plaque and sequenced the V3-V4 region of the bacterial 16S rRNA gene. They compared diversity and community composition between male and female patients. LEfSe identified taxa enriched in each sex. PICRUSt predicted KEGG functional pathways from the sequence data. The Wilcoxon test compared pathway abundance between groups.

Who was studied?

The cohort was 17 patients with severe or advanced (stage III-IV) periodontitis: 11 males and 6 females. All were nonsmoking adults aged 20 to 44 years in China. Mean age was 35.7 years in males and 34.2 years in females, with no significant age difference. Eligible patients had at least 15 natural teeth, no dentures or implants, no recent antibiotics or periodontal treatment, and no systemic disease.

What were the most important findings?

Bacterial richness was higher in males. The Chao1 index and phylogenetic diversity whole tree were significantly greater in males than females (p < 0.01), while Simpson and Shannon indices did not differ. Community structure separated by sex (ANOSIM, unweighted UniFrac, p = 0.001). Females were enriched for the phylum Fusobacteria and order Fusobacteriales, plus the family Tannerellaceae, with Fusobacterium nucleatum noted as female-dominant. Males were enriched for the order Pseudomonadales and family Pseudomonadaceae. Predicted KEGG pathways linked the immune system to women and immune-system diseases to men.

What are the greatest implications of this study?

The results suggest that sex shapes the subgingival microbiome in severe periodontitis, which may reflect differences in immune homeostasis. Sex-specific bacterial patterns could inform how periodontitis is understood and managed. The sample was small (17 patients) and drawn from one center, so findings are preliminary. The design shows association, not causation, and cannot prove that sex differences drive disease severity. Larger studies are needed to confirm these patterns and clarify the underlying mechanisms.
