What was studied?
This study asked whether chronic hepatitis C virus infection, and its progression to cirrhosis, is linked to changes in the gut microbiota. It used a cross-sectional design comparing infected patients with healthy people. Investigators profiled the intestinal microbiota and measured alpha diversity by number of phylotypes and the Shannon diversity index. Diversity was correlated with liver stiffness measured by elastography. Community structure was compared across groups using analysis of similarities.
Who was studied?
The sample was 145 adults analysed in three groups. There were 95 patients chronically infected with hepatitis C virus, of whom 57 had no cirrhosis and 38 had cirrhosis, plus 50 healthy controls without documented liver disease. All participants were human adults. Analyses were also stratified by hepatitis C virus genotype. The design was observational and cross-sectional, capturing each person at a single time point.
What were the most important findings?
Alpha diversity, measured by phylotype count and Shannon index, decreased significantly stepwise from healthy controls to hepatitis C without cirrhosis to hepatitis C with cirrhosis. Both diversity measures correlated negatively with liver elastography. Analysis of similarities showed highly significant differences in microbial communities among the three groups. Stratifying by hepatitis C virus genotype increased these differences further. Distinct genera were either positively or negatively correlated with disease status, giving group-specific abundance patterns.
What are the greatest implications of this study?
The findings indicate that both liver disease stage and hepatitis C virus infection itself are associated with reduced gut microbial diversity and altered community structure. Diversity declined as liver stiffness increased. The authors suggest these shifts could stem from direct virus-microbiota interactions or from indirect effects mediated by the immune system. Because the study is cross-sectional, it shows association rather than causation, and cannot establish the direction of these relationships.
