What was studied?

This case-control study compared the vaginal microbiome of cross-border female sex workers with that of female non-sex workers in China. It aimed to find bacterial differences that might relate to sexually transmitted infection risk. Vaginal swabs were collected and bacterial DNA extracted. The V3-V4 region of the 16S ribosomal RNA gene was sequenced on an Illumina platform. Diversity, community state types, and specific genera were compared between groups.

Who was studied?

The study analyzed 60 women. It included 23 HIV-negative female sex workers (11 Vietnamese and 12 Chinese) from a border region of Yunnan Province, plus 37 Chinese female non-sex workers from a Nanjing clinic. One original sex worker sample was excluded for low DNA. Sex workers were significantly younger (median 25 vs 29, P = 0.001), had more pregnancies (P = 0.012), and more often reported prior vaginitis (P = 0.034). This was a human cross-sectional cohort.

What were the most important findings?

Sex workers had significantly different beta diversity (PERMANOVA statistic 0.321) but similar alpha diversity versus controls. Lactobacillus dominated both groups, with no significant difference in its overall abundance (P = 0.373). Several disease-related genera were more abundant in sex workers. One such genus was detected in 21 of 23 sex workers (91.3%) but none of the controls, and another in 18 of 23 (78.3%). At the species level, several protective Lactobacillus species were lower in sex workers.

What are the greatest implications of this study?

The findings show that commercial sex behaviors track with distinct vaginal bacterial structure and enrichment of pathogens linked to bacterial vaginosis and adverse pregnancy outcomes. This may signal higher infection risk in a hard-to-reach population. The authors stress the small, cross-sectional sample and cannot establish whether sex work or infection alters the microbiome first. They call for larger longitudinal work to identify microbiome profiles tied to sexually transmitted infection risk.
