What was studied?
This study asked how the gut microbiome and its metabolites differ in fatty liver patients who also have colorectal adenoma. Researchers combined 16S rRNA gene sequencing with fecal metabolomics. They compared fatty liver patients with and without adenoma. They used LEfSe to find differential taxa, three machine-learning models to pick metabolite biomarkers, and joint analysis to link microbes to metabolites and pathways.
Who was studied?
The cohort was 58 adults with metabolic dysfunction-associated steatotic liver disease at a hospital in Kunming, China. Thirty had colorectal adenoma (M-CA group) and 28 did not (M-NCA group). The adenoma group was significantly older, at 63.1 years versus 52.2 years. Fecal samples were collected before bowel preparation, and adenoma types included tubular, villous and sessile serrated lesions.
What were the most important findings?
The adenoma group showed clear dysbiosis, with a lower Gut Microbiome Health Index and higher Microbiome Dysbiosis Index. LEfSe identified 75 differentially abundant taxa. Bacteroides vulgatus, Bacteroides ovatus and members of Muribaculaceae were enriched in adenoma cases. The Bacillota to Bacteroidota ratio fell from 14.78 to 6.65. Metabolomics found 116 differential metabolites, and combined modeling narrowed these to 16 biomarkers, led by epigallocatechin.
What are the greatest implications of this study?
This is the first profiling of gut microbes and metabolites in fatty liver patients with colorectal adenoma, offering candidate biomarkers and pathways for later study. Altered lipid, amino acid and bile-acid metabolism may shape a pro-adenoma environment. The authors call the findings exploratory and hypothesis-generating. Age differed between groups and diet and medication were not fully recorded, so associations need validation in larger, matched cohorts before any clinical use.
